We require a copy of your Business License or Resale Certificate. 

*Note this is different from a Health Department License.

Unsure of what you need? 

A Business License or Resale Certificate (and which one your area issues) will be issued by your local government, not the Health Department or Public Health. They are a registration with your local governing body showing that you can conduct business in that jurisdiction.
